Instructions:
  • Do not modify the column headers in Row 1.
  • Every column is a *Required Field*.
  • Enter one club per row.
  • Only include the clubs you are paying for in this order.
Download as CSV:
  1. Open the template and create your copy.
  2. Enter your club information.
  3. Click File → Download → Comma Separated Values (.csv).
  4. Upload the CSV file below.
Quantity Required: The product quantity must match the number of club rows in your CSV file.

Example:
10 clubs in the spreadsheet = Quantity 10.
